Are you transferring your membership within Texas from one local society to another?

Please contact Membership Coordinator Rachael Daigle at 512-443-3675 ext 137 or email.

Please include your new office and home addresses, phone numbers, and fax numbers in the body of the email.

Are you moving into Texas from another state?

Please fill out a Tripartite Membership Application and a representative from the Department of Member Services will contact you.

Are you moving out of Texas and want to transfer your membership to another state dental association?

Please contact Membership Coordinator Rachael Daigle at 512-443-3675 ext 137 or email, with your new mailing address.
